Once you are registered with Social Security as an autónomo, your right to public healthcare is recognised automatically. **You then ask for your Basque health card (*TIS*) at your health centre.**

## Before you start

- [Your Social Security registration](https://autonomo.eus/steps/register-social-security)
- [A padrón certificate (*certificado*, not a *volante*)](https://autonomo.eus/steps/padron)
- Your TIE or EU registration certificate, and your passport or national ID card
- [Cl@ve](https://autonomo.eus/steps/clave) or [your FNMT certificate](https://autonomo.eus/steps/fnmt-certificate), if you want to download the Social Security document online. There is also a way without them (step 1).

## What it unlocks

- Care from Osakidetza, the Basque health service
- Prescriptions at any Basque pharmacy. A digital TIS has worked in all of them since June 2024. You get it in the Basque Government's NIK app, with your [BakQ](https://autonomo.eus/steps/bakq). See [the digital TIS](https://www.euskadi.eus/tis-digital/web01-a2inform/es/). Keep the plastic card too.

## How to do it

1. Get the document that recognises your right to public healthcare (*documento acreditativo del derecho a la asistencia sanitaria*) from the Social Security electronic office (*sede electrónica*). Choose one way:
   - **With Cl@ve or your FNMT certificate:** log in to [Tu Seguridad Social](https://sede-tu.seg-social.gob.es/). Scroll down to the "Asistencia Sanitaria" (healthcare) section, check your right and download the document. Print it. The Social Security website calls this service ["Asistencia sanitaria. Consulta del derecho y Alta de beneficiarios"](https://sede.seg-social.gob.es/wps/portal/sede/sede/Ciudadanos/Informes+y+Certificados/202044) (check your right and add beneficiaries).
   - **Without Cl@ve or a certificate:** use the service ["Asistencia sanitaria: Consulta del derecho y emisión del documento acreditativo" (sin certificado)](https://sede.seg-social.gob.es/wps/portal/sede/sede/Ciudadanos/Informes+y+Certificados/202044SinC) (check your right and get the document, without a certificate). Click "Consultar", fill in your details and tick the box to get the document. You get a reference number. Come back with it after at least 15 minutes to get the document. If you give an email address, the answer also comes by email. It stays available for 30 days.
2. Find your health centre (*centro de salud*). It is the one nearest your home. Use the [Osakidetza health centre finder](https://www.osakidetza.euskadi.eus/buscador-de-centros-sanitarios-y-hospitales/webosk00-cercon/es/): choose the type of centre (*Tipo de centro*), your territory (*Territorio Histórico*) and your town (*Municipio*).
3. Go there in person and ask at the desk for the TIS application form (*formulario de solicitud*). You fill it in at the centre.
4. Hand in your ID, your padrón certificate and the Social Security document. For children, also bring your family book (*libro de familia*).

## Where

Your nearest health centre. Find it with the [Osakidetza finder](https://www.osakidetza.euskadi.eus/buscador-de-centros-sanitarios-y-hospitales/webosk00-cercon/es/). For all Osakidetza phone numbers, see [Contact Osakidetza](https://autonomo.eus/steps/offices#contact-osakidetza). For questions about the card, write to the address for your territory.

## Your family

You can add your spouse or registered partner, and children under 26, as your beneficiaries. Do it online on the Social Security website or at a Social Security office (*CAISS*) by appointment (901 106 570 or 915 412 530). Bring proof of the relationship, such as a marriage or birth certificate.

- **Online:** log in to [Tu Seguridad Social](https://sede-tu.seg-social.gob.es/) and go to the "Asistencia Sanitaria" section. The service is ["Asistencia sanitaria. Alta de beneficiarios"](https://sede.seg-social.gob.es/wps/portal/sede/sede/Ciudadanos/Asistencia+Sanitaria/202043_TUSS) (add beneficiaries). It asks for a stronger login than the check in step 1, and shows the login methods you can use when you start.
  - A baby under 3 months is added at once, and you can download the document straight away.
  - For anyone else, you download a receipt (*justificante*). Social Security sends its decision (*resolución*) to your home address by post.
- **At a CAISS office:** book an appointment online with ["Cita previa para prestaciones y otras gestiones"](https://sede.seg-social.gob.es/wps/portal/sede/sede/Ciudadanos/cita+previa+para+pensiones+y+otras+prestaciones/13cita+previa+para+pensiones+y+otras+prestaciones) (appointments for benefits and other procedures), or by phone on 901 106 570 or 915 412 530. Bring [form 6-071](https://www.seg-social.es/descarga/es/46493), filled in as a holder adding beneficiaries (*Supuesto B*), each person's ID or TIE, and proof of the relationship: your family book, marriage certificate, partnership register certificate or birth certificate. For all offices, see [Contact Social Security](https://autonomo.eus/steps/offices#contact-social-security).

When you have your family member's document, ask for their TIS at the health centre the same way.

## Cost and time

- Cost: the card is free. What you pay for prescriptions depends on your income.
- Time: without a padrón certificate, the application can be delayed by at least 2 months.

## Before you are covered

In an emergency you are always treated. Call 112 if you cannot get to a hospital. Keep private health insurance until your right to public healthcare is recognised.

- **Emergency:** go to a hospital emergency department (*urgencias*) or your health centre. See [Osakidetza emergencies](https://www.osakidetza.euskadi.eus/emergencias/webosk00-emecon/es/).
- **Health advice by phone, 24 hours:** call Osakidetza's advice line (*Consejo Sanitario*) on 900 203 050.
- **If Social Security cannot find you or does not recognise your right:** the result of step 1 tells you why. Call the Social Security information line (INSS) on 901 166 565 or 915 421 176, Monday to Friday, 9:00 to 20:00. The [Basque Government leaflet on access to the health system](https://www.euskadi.eus/contenidos/documentacion/acceso2026_sistema_salud/es_def/FVMA-Diptico-Comision-Salud-Cas.pdf) explains the other routes to care in Euskadi.

## Common mistakes

- Going without the padrón certificate. Bring it even where it is optional for adults. Adults must bring it if their town is on the [list of towns whose padrón the Basque health department cannot check](https://www.euskadi.eus/contenidos/informacion/dac_tarjeta_sanitaria/es_def/adjuntos/ANEXO-tis.pdf).
